If you look at the screenshots of steps 4 and 5, you can already see the Bank, Sub-Bank and Program fields. The following image clearly points out these fields.
These are a per-clip setting and you use these to change programs and banks on your synth. How it works exactly differs a bit per synth. For example, some synths only use programs 0 up to 99 (probably displayed as 1 up to 100) and then use the value of bank to select the correct bank (0 = A, 1 = B, etc). Play around with these fields and you'll see how your synth reacts.
Some synths just number their programs without mentioning banks, starting at 1 up to a large number, like 1024. Program 0 in the synth might correspond to program 0 and bank 0 in Live, program 130 would then correspond to bank 1 and program 2; 128 programs per bank, so first see how many times the number 128 fits in the program number and then subtract that number (times 128) from the program number: 130 - 1 x 128 = 2.
Just giving it a try is probably the quickest way to get familiar with this. What you can also do is start a program on your computer (MIDI Monitor on a Mac, MIDIOX on Windows) and then change programs on your Nord; you'll see what MIDI messages the Nord sends when you do this (program, bank MSB and bank LSB). Bank MSB = bank, bank LSB = sub-bank (I think, but maybe it was the other way around). For this to work, you do need to have the Nords MIDI OUT connected to your computers MIDI IN port.
Switching to performance mode cannot be done via MIDI (I'm quite sure of this, but not 100% - I needed this myself at one point), so you need to manually switch the synth to performance mode.
