The manual is not very clear about the output routing options, but it is possible to route audio to individual outputs. So there is definitely a possibility that the output is routed just to output 1. it would be highly unusual for 3 of the 4 outputs to just suddenly fail, so I'd just dig through the system settings and see what the output routing is set to.

SLOT ROUTING SETTINGS ARE A.12 / B.12 / C.12 / D.12 But OUTPUT 1 and 2 are left Sound no right.
And OUT 3-4 No Sound. Heaphoneout are stereo and louder.
