but I guess I still have to sacrifice 1 of the 3 NS4 synths or not?
Where does the MIDI Program Change "come" from then
Yes, I believe you sacrifice the 3rd NS4 synth to instead function as the Extern section on the NS3 did, that's presumably where the PC would be specified.
In effect, that particular new feature of the NS4 (having a 3rd synth sound) is not available when you are using Extern.
There are people who would have gladly given up the Extern features of the NS3 if they could have had a 3rd synth. They are the people who will be happy with this feature. For other people, if there are not enough other benefits, then you could be better off with an NS3. Just as there were people who stuck with the NS2 because, for all the advances of the NS3, there were still things the NS2 did better.

Maybe the synth layer is not lost entirely (if you only need to send a PC message on load). I asume that when the synth layer is switched to exern mod, NS4 still remembers its settings for samples/analog mode. If so, you just need to save the program with one layer in extern mode. Upon loading the program, such layer will send the required PC message. Then you can manually switch it back to the normal mode (samples or analog) and play. Not ideal but doable. Or have I missed something?

Yes, I was also thinking that this could be one of the compromise options ...

Has anybody a factory preset list of the Nord Stage 4? I wonder if the FM Piano Sounds will be better in the NS4? Currently I am using a NS3 and a Kronos, but I would love to cover all sounds I need with just the NS4. I need the Kronos for Moog-, FM- and especially for muted and distorted Guitar Sounds.
The sounds may be new - the more as there are some new effects - but the basic samples (synth and piano) will probably all be the same as before. At least the same as for the Piano 5.

A list of patch names won't tell you much. But since the NS4 synth engine is based on the Wave 2 rather than the A1 (which the NS3 was based on), it might be interesting to check out the sounds of the Wave 2 (e.g. youtube videos) to see if that synth might have FM and VA sounds that are more like what you're after.

Just the J3PO ones but I assume these patches shown will be available as factory presets

though the NS4 has a synth engine that the Piano 5 lacks, and this Wave 2 based synth engine has FM synth capabilities. Whether it has better or worse actual FM pianos than the sample-based FM pianos in the piano library, I don't know.
