Not sure if something like this is possible with the stage 3. I can always sequence it or sample, but trying to think if there was a way to recreate on the fly.
playing the A/E in left hand and right hand the little melody, but out of hands to also apply the creepy pitch bend. Can anything else be setup to modify the pitch?
Using the synth engine with a stock sample (176 toy piano).

Unfortunately only the pitch bar can be used with the sample, however the pitch can be morphed to the Control Pedal for the Classic, Wave, and Formant waves available. I have an example posted at nord-stage-3-programs-ns3p-ns3pb-files-f32/find-sound-nick-semrad-prophet-08-rev2-t16159.html#p106971 where the pitch and filter FREQ are morphed to the Control Pedal. You could try modifying the Tubular Bells wave to see if you could emulate the toy piano. I would go at it with both panel synths and use the 2nd to get the initial striking sound (perhaps toy piano sample cut off with almost immediate decay). Take care

Following you advice, here's what I came up with.
Using the control pedal to roll off the pitch and freq, also having it bring in some ring modulation and push it farther into the reverb.

That worked pretty well! I like how you start with the toy piano and then blend in the effect.
