Please have a look NL4 manual page 47. Bank should be selected by CC00, CC32 and followed by immediate program change message.

I can't figure out how the midi values for program and bank map to the values I see on the Nord Lead itself. For example I saved something in performance mode to program D25. Which bank/program midi signals do I send to use that program?

How are you sending the midi information? From software? If so, which? I have a Max/MSP/M4L code snippet that can do that for you if you like.

I'm sending from a Synthstrom Deluge. It can send a Program and a Bank, each of which is a number from 0-128.

As Jeltz said already, you need to send CC0 (Bank MSB) followed by CC32 (Bank LSB) followed by the program number, exactly in that order and one immediately after the other with no intervening MIDI commands. Many sequencers, for unknown and unconscionable reasons, do not support the MIDI standard (and required) Bank LSB controller 32 and only send CC0. This will not work for Nord products among many other properly-implemented MIDI boards (some DSI as well). If your sequencer cannot send sub-bank aka CC32 aka Bank LSB, there is no way it will be able to choose the proper patch destination on your Lead or any other synth that uses Bank LSB for patch selection.

Elektron's sequencers are notorious for this problem as well and they don't seem to give two f***s about fixing them either. If Synthstrom can't be arsed to support the MIDI standard correctly, go complain to them. I'm told their software team is slow but responsive.

The Deluge has proper Implementation to do what you want. Let me know if this is still an issue to be solved.
