Anyone using this model mixer know if the headphone jack output gives a true stereo sound or is it summed to mono? I can't get any support (phone number or email contact for a question) from their website.
I've tried many different ways to troubleshoot this problem, but I'm finding it's not a stereo output. I just get a summed sound, no real separation. The most obvious way I tested it was with my Nord's Rhodes sound with the ping pong effect (pan). I didn't hear the effect. It works great from the main outputs into two speakers.

Just in case... Do you have the chnnels fully panned in opposite directions?

Yes, the headphone output is stereo. This is clearly shown in the block diagram in the instruction manual. No doubt about it.

Yes, the channels are panned in the opposite directions (left and Right) when I'm sending my left and right keyboard outputs into two of the 1st two of the four mono channels, inputs channels one and two. When I use the stereo channel(s) (ST1 or ST2), the balance knob of the channels will be in the center, for equal sound from both the left and right inputs from the keyboard.
Another way I tested this was plugging my Ipod into ST1 channel (the one that accepts the RCA input for Ipods etc.) I played a song "Jimi Hendrix's, ''Castles Made Of Sand' ". This song has a lot of panning of it's sound. When listening through the headphones, through the mixer, I got the music out of each side of the headphones speaker, but I wasn't hearing those panning effects that are so noticeable on that recording. Also when using the headphones directly into the keyboard jack of the Nord, I hear the separation of those stereo sounds of the keyboard.

I'd say that the two channels are accidentally short-circuited somewhere, either in the headphone output of the mixer or in in the cable. Have you tried a different pair of headphones?

Yes. I even went to Guitar Center and tried their zed 10fx with their headphones and came to the same conclusion.

Also I noticed that the pan knobs and particularly the balance knob in the two stereo channels have no effect of sending the sound from one headphone speaker to the another. It's just constant sound from both speakers.

Sounds like a faulty unit to me... unless your headphones are faulty.
What happens if you connect only one channel? Can you pan it?
What happens when you use the AutoPan effect, do you see a difference in volume in the led levelmeter?

Just some ideas from someone who has no idea about that mixer:
Do the headphone-related switches make any difference? I.e. what happens if you use the "Listen" switch of a particular channel, vs. listening to the entire main mix?
What about the monitor output? Is it in stereo? Does that depend on the monitor source (phones or main mix)?

To amplify what FunKey has said, if you have ANY of the Listen buttons on each channel depressed, the output will be mono. Only if all are unpressed will the headphone output be stereo (theoretically).
I would also try unchecking all the Phones Select buttons. Then, if problem not solved, select Record (on Phones select) and press the Record buttons on the channels you want to monitor. The Record bus is stereo, Pre Fade Listen (PFL) is mono...
